ABOUT DIAMOND HEART LAB SERVICES, LLC

· Diamond Heart Lab Services, LLC (DHLS) is responsible for drawing and delivering specimens to the lab for processing. DHLS does not collect ANY type of specimen without the order from a Physician or a lab labeled requisition form.  We do not give any medical advice, treatment or diagnose any patient.

· DHLS will handle all deliveries of samples to the lab.  If a sample is being collected for a Laboratory that needs to be picked up via courier service, DHLS is not responsible for any delay of arrival to the laboratory once the sample has been delivered or picked up by the courier service.

· Before arrival for any appointments, DHLS will need proper information collected, i.e., patient ID, copy of insurance card (front and back) and lab orders.  This information will need to be submitted to our office within 48 hours prior to appointment time to ensure accuracy for the staff. If information is not received within the allotted timeframe, the appointment will need to be rescheduled until all proper information is provided.

· DHLS is not responsible for any errors that may occur at the laboratory. It is the laboratory’s responsibility to process specimens once received by DHLS. In the event of a lab error, DHLS will gladly return to recollect the sample at a discounted rate. Any error made by DHLS will be corrected at no extra charge. 

· Any billing questions or problems for tests collected for this service should be directed to your insurance company or the laboratory billing department.

· Payment for services provided is collected the day before or the day of service.

· DHLS is an independent convenience service. Our company is not affiliated with any Laboratories, Doctor’s office, or hospitals. DHLS does not have authorization to give discounts for any lab test that are being collected.

· DHLS does not bill insurance companies, any claims must be submitted by the client, with no guarantee of reimbursement.

· DHLS will not provide personal information to any unauthorized individuals or entities unless written authorization is provided by the patient to DHLS.

· All client information is always kept private and confidential.

· DHLS does not receive results on behalf of our clients.  Any results for any tests will need to be retrieved from your physician, who is the responsible party where the results are sent.

Cancellation Policy:

If an appointment needs to be canceled, a 24-hour notice will need to be given to DHLS so that the appropriate steps can be taken with the staff and another appointment can be made in a timely manner that is convenient for you and the company.  

All cancellations will be charged an inconvenience fee of $25 to reschedule.  If an appointment needs to be rescheduled, please send a request to reschedule via email to info@diamondheartlabservices.com. Please include your name, original appointment time and address in the email.
